Children's and young peoples Continuing Care,covering Buckinghamshire, Oxfordshire and Berkshire West (BOB) are looking forassessors with a range of clinical skills to support assessments of Childrenand young people with varying but complex health needs. If you have experiencein complex physical health and /or mental health and/or learning disabilities,you may be just the person to join our thriving team. We are looking forindividuals who are motivated to use their clinical expertise to make a positivedifference, delivering assessments using the National Framework and seeking CYPand family based solutions often through Personal Health Budgets.

Main duties of the job

As we cover a large geographical area therewill be opportunity to have a primary base in one of our counties however therewill also be an expectation that work is carried out across BOB in answer toreferral receipt and clinical need. Our current assessors primarily workvirtually however there is an expectation that face to face assessment andvisits to home and school are part of each assessment.

There will be ample opportunity to develop new skillsworking alongside existing learning disability, mental health and physical health case managers leading theway in the application of Childrens Continuing Care Framework to supportequitable, good quality health care delivery and positive health outcomes.

Job responsibilities

Using clinical expertise in applying the Continuing HealthCare Framework for Children, the post holder will be responsible for Assessingand Reviewing individuals within or referred into CYPCC.

The post holder will work with wider teams to support thedelivery of individualised care delivery including Personal Health Budgets(PHBs).

Working within the remit of the National Framework (NF) for CYPCC,the post holder will provide a high quality service that supports the deliveryof an effective and competent service that consistently delivers anindividual-focused intervention with patients, their families andrepresentatives.

Take a lead on ensuring transitionprocesses are continually developed and improved to achieve seamless CYP keytransition milestones.

The post holder will be required to have a good knowledge of healthand social care practice and be able to demonstrate an understanding of CYPCCand be prepared to extend their skills, knowledge and competence in this field.

This post will require good communication skills withindividuals and their families or representatives and requires the ability towork in a multi-disciplinary way with colleagues.

Deliver against objectives, achieving quality outcomes,prioritising own workload and working to tight deadlines.

The post holder will be responsible for ensuring theirprofessional registration and mandatory training is up-to-date
