Job Title: Leaving Care Personal Advisor Officer

Location: Wakefield (Hybrid)

Pay Rate: £16.10 per hour

Hours: Full-time, Monday to Friday, 09:00–17:00

Contract Type: Temporary

Driver: Yes

Role: To support vulnerable young people transitioning from care by managing complex cases involving assessment, planning, intervention, and coordination of services, aligned with the Children Act, national guidance, and local policies to help these young people achieve positive, independent futures.

Requirements:

• GCSE Grade C or above (or equivalent) in English and Maths. Relevant professional qualification.
• Full UK driving licence (or reasonable adjustments for disabilities).
• Understanding of child development, care leavers' challenges, and familiarity with relevant legislation and safeguarding procedures.
• Experience working with diverse young people, building supportive relationships, developing intervention strategies, and managing challenging behaviors.
• Proficiency with IT systems and digital recordkeeping.
• Strong interpersonal skills, commitment to support and customer care, organizational skills, and dedication to equality and safety.

Main Tasks:

• Manage caseloads, conduct assessments, planning, and reviews.
• Collaborate with multi-agency partners in reviews, planning, and safeguarding forums.
• Maintain accurate case records and produce reports.
• Provide practical support including life skills, accommodation, and transport assistance.
• Apply safeguarding principles and promote inclusion.
• Participate in supervision, team meetings, and training.
