Neighbourhood Integration Lead | Homerton Healthcare NHS Foundation Trust

This role leads the operational implementation of Integrated Neighbourhood Teams (INTs) across 2 designated neighbourhoods serving 30,000-50,000 residents each. Working under the Head of INT, you'll establish a "team of teams" approach, bringing together primary care, community health, mental health, social care and voluntary sector partners into cohesive neighbourhood units. The position requires hands-on leadership to transition from existing multidisciplinary meetings to fully operational integrated teams with shared accountability for population health outcomes. You'll work closely with established neighbourhood structures (Forums, Leadership Groups, coordination teams) to ensure INTs are grounded in local community knowledge and needs. Key focus areas include frailty, children/young people, long-term conditions and mental health pathways. This is one of 4 roles working collaboratively to operationalise INT across City and Hackney while allowing local adaptation to neighbourhood-specific needs. Both clinical/practitioner expertise and project management skills are essential for success in this transformational healthcare delivery role.
Neighbourhood Team Development:Operationalise agreed INT models, facilitate shared operating protocols and care pathways enabling coordinated delivery across organisational boundaries. Support population health management approaches and facilitate co-location opportunities.
Partnership Coordination:Build effective relationships with Primary Care Networks, coordinate with Hackney and City of London social care teams, engage voluntary sector partners as core team members, work with local public health teams and neighbourhood leadership groups.
Pathway Leadership:Coordinate pathway development for assigned clinical priority areas across neighbourhoods, share expertise with other Integration Leads to ensure common approaches.
Operational Management:Identify and escalate system-level barriers, coordinate practical solutions to integrated working challenges, provide leadership to neighbourhood coordination roles using matrix management, present complex information accessibly to diverse audiences, work on procurement and contract monitoring.
Professional Development:Maintain knowledge and skills, participate in appraisal processes, keep updated with professional issues and health/care landscape, represent Trust at forums, maintain awareness of Trust initiatives and policies.
