Are you an experienced administrator with strong organisational skills and a calm, professional manner? We're looking for a Pathway Coordinator to join our Care Division on a temporary basis, supporting the delivery of Continuing Healthcare (CHC) services across the UK.
This is a full-time, fully remote opportunity, ideal for someone who can start immediately and hit the ground running.
About the Role
As a Pathway Coordinator, you'll provide vital administrative support to our Care Practitioners, helping to coordinate CHC reviews, manage sensitive case data, and ensure accurate and timely communication with patients and representatives. You'll handle appointments, documentation, and queries with professionalism and empathy, ensuring a smooth and supportive pathway for all individuals in our care.
Key Responsibilities
Manage and prioritise incoming CHC case data in line with protocols.
Schedule review appointments, issue letters, and handle rebookings if needed.
Communicate sensitively with patients, families, and representatives via phone and email.
Ensure accuracy and compliance in all documentation and data handling.
Support Care Practitioners by preparing assessment documents and monitoring inbox traffic.
What We're Looking For
Previous administrative experience, ideally involving sensitive or confidential information.
Excellent customer service and communication skills, especially in emotionally sensitive contexts.
Strong organisation and time-management skills, with the ability to prioritise.
Confident using Microsoft Word, Excel, Outlook, and Microsoft Teams.
Understanding of GDPR and confidentiality standards.
Minimum of 5 GCSEs including English (Grade A-C or equivalent).
Experience working in Continuing Healthcare (CHC) or a healthcare setting.
