Do you have proven experience implementing or supporting a Patient Safety Incident Response Framework (PSIRF) with demonstrable experience in conducting or overseeing patient safety investigations?

As the Patient Safety Manager, you will support the Group Quality Assurance Director and the patient safety team to ensure effective collaboration between regional and corporate patient safety functions, uphold Elysium PSIRF policy, and foster a culture of continuous learning and improvement.

Responsibilities

Patient Safety Management

  • Support the daily management of patient safety processes and investigate patient safety incidents in line with policy and PSIRF.
  • Raise risk concerns to the Group Quality Assurance Director and the Patient Safety Team.
  • Ensure the mortality tracking process is kept up to date.
  • Work with the legal team to support mortality review and other legal matters as required.

Incident Reporting & Investigation

  • Oversee thorough investigations into patient safety incidents using PSIRF.
  • Provide support to those undertaking patient safety investigation using the SEIPs model.
  • Review responses to patient safety incidents, suggest adjustments to plans or strategies, and request further evidence as necessary.
  • Analyse and review escalated Patient Safety Events via P1 and P2 reports at weekly meetings, including assessing appropriate learning responses.
  • Critically evaluate and analyse complex, diverse, and sometimes contradictory evidence from a wide range of sources.
  • Review Patient Safety trend information in collaboration with the Patient Safety Team.
  • Obtain specialist advice when required from the Patient Safety Team.

PSIRF Implementation & Learning

  • Support services to implement and deliver PSIRF and collaborate with relevant services and clinical networks to embed learning from patient safety events.
  • Support the delivery of PSIRF‑related training.
  • Provide expert advice to all staff on matters relating to PSIRF.
  • Review and consider learning from external sources such as national enquiries and organisations like HSSIB, identifying where learning applies to Elysium services.
  • When required, chair and facilitate PSIRF Quality Summits.

Reporting & Governance

  • Oversee investigators, review final reports, and support quality checking prior to circulation.
  • Support the patient safety team with collation of information for reports required internally and externally.
  • Provide and receive complex, sensitive, and contentious information and present as appropriate to the patient safety meeting.
  • Ensure compliance with all statutory reporting (including Duty of Candour).
  • Contribute to regular reports from the Patient Safety Workstream to Clinical Corporate Governance, the Elysium Management Board, Risk Committee, and Ramsay Healthcare.
  • Support the development and timely delivery of all Elysium board reports relating to Patient Safety and Mortality.

Qualifications

  • Proven experience in implementing or supporting PSIRF or related patient safety, risk, or governance frameworks.
  • Demonstrable experience in conducting or overseeing patient safety investigations.
  • Experience engaging with service users, carers, or staff to support learning and improvement following patient safety events.
  • Experience coordinating multidisciplinary teams in quality, safety, or risk management initiatives.
  • Experience analysing patient safety data, trends, and developing improvement action plans.
  • Experience in NHS or independent sector mental health, learning disability, or hospital environments.
  • Experience delivering training or facilitating workshops on patient safety, incident management, or governance topics.
  • Relevant degree or professional qualification and/or substantial equivalent experience in a healthcare or related setting.
  • Evidence of continued professional development in patient safety, risk management, governance, or quality improvement.
  • Training in investigation methodologies and/or significant experience leading incident investigations.
