We provide tailored, tech-enabled primary care services through a fully managed, pharmacist or technician-led model suited to Primary Care Networks (PCNs), GP practices, and federations, aiming to reduce workloads and streamline processes.

Main Duties

• Possession of an independent prescribing qualification (not essential for non-prescribing roles).
• Qualification or enrollment in the CPPE Clinical Pharmacist Training pathway, offered to PCN pharmacists to enhance clinical skills.
• Strong clinical leadership, communication, and interpersonal skills, with a good understanding of PCN Pharmacist roles.
• Contributing to health outcomes improvement in long-term conditions.
• Providing expert advice on pharmaceutical matters.
• Reviewing patients' acute medication requests.
• Completing discharge and clinic letters.
• Conducting structured medication reviews as a PCN Pharmacist.
• Supporting patients with self-limiting conditions.
• Supporting practice achievement of Impact and Investment Fund and QOF targets as a GP or Practice Pharmacist.

The successful candidate will be a practicing Pharmacist with prior experience in clinical pharmacy roles, dedicated to delivering excellent care.
