We are looking for dedicated Nurses to join our Community Hospitals teams across Kent. You'll oversee the holistic care provided to patients, provide leadership by maintaining clinical standards, and may take charge of the ward in the absence of senior staff. Our units predominantly provide rehabilitation and end-of-life support for palliative patients and their families.
You'll work closely with patients, families, and a multi-professional team including doctors, physiotherapists, and occupational therapists. You'll contribute to a culture of excellence and be part of a team committed to providing safe, effective services and a positive patient experience. - Hawkhurst Community Hospital, TN18 4PU
- Tonbridge Cottage Hospital, TN11 0NE
- Victoria Hospital, Deal, CT14 9UA
- Whitstable and Tankerton Hospital, CT5 2HN

Driving may be required for these roles as you may be asked to travel to other sites to provide support to your community hospital colleagues if and when needed.
Main duties of the job

We treat the whole person, not just the condition, through practitioner-led and therapy-focused work. Caring for people in the community requires specialist knowledge and skills, which we will develop through our excellent educational opportunities, ranging from degrees to apprenticeship schemes.
We expect you to maintain high clinical standards, use evidence-based practises, and contribute to continuous improvement. You will manage a caseload of patients with complex needs.
You'll be responsible and accountable for the holistic care provided to patients. You will carry out assessments, develop integrated care packages, support junior staff, and actively participate in a multi-professional team to ensure high quality care is provided to our patients. You will be expected to participate in 24-hour internal rotation and move to other community hospital sites to cover the needs of the service.

Person Specification
Ability to manager
- Able to support the implementation of local and national agenda for health under the guidance of the team lead / ward manager.
Qualifications
- Registered Healthcare Professional with relevant Professional body 1st Level Nurse/ NMC Degree in a health relevant subject or willingness to complete a degree
Knowledge
- Awareness of professional issues and recent developments in the NHS Knowledge of up to date clinical practice
Skills & abilities
- Ability to organise and prioritise work, using own initiative
Personal attributes
- Ability to motivate and organise others to ensure best practice.
Ability to manage
- Ability to prioritise care delivery in a professional manner, taking into account Trust policies and procedures.
