This role has a starting salary of £42,239 per annum, based on a 36-hour working week. We welcome both full-time and part-time applications.

We are hiring a Social Care Practitioner to join our Transition team.

About the Role

You will collaborate with individuals, carers, agencies, and support brokers to support vulnerable adults and their families, promoting independence and choice. You will manage a caseload with complex needs, undertake safeguarding enquiries, and conduct community assessments.

You will work in a team with professional supervision and supportive colleagues.

Shortlisting Criteria

Applicants must have a valid UK driving licence and access to a vehicle, with willingness to travel. Reasonable adjustments will be made for disabilities.

The current contractual location is Quadrant Court, with plans to move to Victoria Gate, Woking later this year.
