About the Role: As a Social Worker within the Neighbourhood Team, you will:
- Work collaboratively to promote independence and wellbeing
- Complete strengths-based assessments, safeguarding enquiries, and care planning
- Support adults with complex needs, including physical disabilities, mental health issues, and learning disabilities
- Build relationships with health, housing, and voluntary sector partners
- Contribute to the continuous improvement of local services
- Social Work England registration
- Must hold qualification in Social Work
- Enhanced DBS
- Experience working with adults in statutory settings
- A passion for person-centred, strengths-based practice
- Strong assessment, communication, and risk management skills
