Base pay: £55,000 per annum.

Location: Carrfield Medical Centre, Sheffield.

Job type: Permanent. Hybrid working (3 days onsite, 2 days home) after successful probation.

Working hours: Monday – Friday 8:30AM – 6:30PM.

Role Overview

The Advanced Clinical Practitioner (ACP) will provide autonomous, expert‑level clinical care, managing patients across a range of acute and chronic conditions. The ACP plays a key role in reducing unnecessary GP referrals, ensuring patients receive timely and effective care, and developing long‑term relationships with patients to support optimal health outcomes.

Key Responsibilities

  • Assess, diagnose, and manage patients with acute and chronic conditions.
  • Order, interpret, and act on investigations (blood tests, imaging, ECGs, spirometry).
  • Prescribe and review medications independently.
  • Perform minor procedures (wound care, sutures/staples, joint injections if trained).
  • Develop and implement individualised care and treatment plans.
  • Manage long‑term condition clinics (diabetes, hypertension, COPD).
  • Provide health promotion, disease prevention, and lifestyle advice.
  • Triage patients and decide on escalation or referral to GPs or specialists.
  • Provide patient education, self‑management support, and follow‑up care.
  • Contribute to service planning and improvement initiatives.
  • Support clinical audits and quality improvement projects.
  • Support practice in implementing evidence‑based guidelines and protocols (NICE, local policies).
  • Provide clinical leadership to junior staff, nurses, or other allied health professionals.
  • Use data and insights to improve patient outcomes and service efficiency.
  • Maintain accurate and detailed patient records.
  • Ensure compliance with data protection, clinical governance, and safeguarding policies.
  • Participate in revalidation, CPD, and professional development activities.
  • Contribute to multidisciplinary team meetings and case discussions.

Essential Criteria

  • Registered Nurse, Paramedic, or relevant healthcare professional with NMC/HCPC registration.
  • Meets the standards for Advanced Clinical Practice (ACP) at advanced level.
  • Qualified Independent Prescriber.
  • Meets professional revalidation requirements.
  • Master’s degree in Advanced Clinical Practice.
  • Experience in prescribing and conducting medication reviews.
  • Disclosure Barring Service (DBS) check.

Desirable Criteria

  • Experience working in primary care settings.
  • Additional clinical qualifications or specialisms.
