Overview

We are seeking a highly skilled Advanced Nurse Practitioner to join our Clinical Assessment Service (CAS), supporting patients across Surrey. This pivotal role involves assessing patients remotely via telephone, planning and administering treatment, and making timely decisions regarding discharge or referral, all within your clinical scope of practice. You will be a key part of a multi-disciplinary, fast-paced environment, operating in line with GP surgeries and Practice Plus Group protocols.

In this role, you will manage your own virtual caseload, providing high-quality, evidence-based patient care, including prescribing where appropriate. You will work closely with other healthcare professionals to ensure patients receive the most appropriate outcomes—whether that's self-care advice, or referral to face-to-face services. The role offers a unique opportunity to demonstrate clinical leadership, contribute to service development, and support continuous improvement through clinical governance and audit. Responsibilities

  • Receiving and assessing telephone enquiries from patients, using agreed clinical protocols to provide advice, treatment, and referrals
  • Managing a virtual caseload, making autonomous clinical decisions within your competency
  • Supporting and liaising with other clinicians and specialist teams to achieve the best patient outcomes
  • Maintaining accurate, timely records using computerized clinical systems
  • Promoting patient self-care and educating patients on appropriate use of healthcare services
  • Participating in audits, clinical governance, and service development activities
  • Facilitating effective communication and clear patient pathways across primary, community, and secondary care settings
  • Providing clinical supervision and supporting junior staff as required
  • Complying with professional codes of conduct, policies, and safety standards

Qualifications

  • Registered Advanced Nurse Practitioner (MSc) with independent prescribing qualification and current NMC registration
  • Proven experience in telephone triage, clinical assessment, or urgent care environments
  • Strong clinical decision-making skills and the ability to work autonomously within a multi-skilled team
  • Excellent communication and interpersonal skills, with a patient-centred approach
  • Ability to work effectively under pressure in a fast-paced setting
  • Commitment to continuous professional development and maintaining clinical competence
  • Flexible attitude towards working hours, including evenings, weekends, and bank holidays
  • Leadership qualities, including the ability to motivate and support colleagues
  • Understanding of medico-legal responsibilities and clinical governance frameworks
