Job Title:Advanced Practitioner – Transitions Team
Location:Nottinghamshire
Rate: £43.53 per hour

We are seeking an experienced and dedicated Advanced Practitioner to join our Transitions Team based in Nottinghamshire.

Primary Responsibilities as an Advanced Practitioner:

• Provide high-quality support and guidance to young adults transitioning from children’s services to adult services.
• Work collaboratively with multi-disciplinary teams to ensure a smooth transition process.
• Conduct assessments and develop personalized care plans.
• Monitor and review the effectiveness of care plans and make necessary adjustments.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK license
• Post-qualified experience
• Degree level or equivalent in Social Work
