About the Role

A council is seeking experienced Advanced Practitioners to join their Older Persons Team. The role involves delivering high-quality statutory social care, including safeguarding, assessments, supervision, and developing innovative community solutions. You will work closely with multi-agency partners to ensure statutory responsibilities are met, safeguarding the well-being and safety of the community.

Responsibilities

• Deliver statutory social care services and safeguarding
• Conduct assessments and supervision
• Develop community-based solutions
• Collaborate with partner organisations, professionals, individuals, families, and carers
• Ensure compliance with health and social care standards

Qualifications & Experience

• Qualified social worker or occupational therapist
• Minimum of 2 years post-qualification experience
• Active in specialist roles such as Moving and Handling Practitioner, Practice Educator, Approved Mental Health Practitioner, or Best Interest Assessor, or willing to undertake one within the first year
• Strong communication skills
• Valid driver’s license and access to reliable transport
