ANP or Paramedic ACP (Prescribing): Outreach Team Co-ordinator

Arch is an outstanding healthcareprovider delivering primary care to homeless and vulnerably housed patients inBrighton & Hove. In addition to a specialist GP surgery, we also provide in-reachservices into the Royal Sussex County Hospital and a variety of communitycontexts which serve people who are without secure accommodation. We are acommitted and experienced team with a passion for improving the health andwell-being of people who are homeless or vulnerable.

We are looking to appoint a prescribing ANP or Paramedic ACP to lead our enhanced outreach service, which delivers healthcare on the streets, in day centres, hostels and emergency/temporary accommodation. This project was set up in October 2022 and requires staff to demonstrate a high level of flexibility, drive and ambition. In this role, you will work closely with a GP, Health Engagement Workers and other members of the Arch team to ensure the success of the project. The project is supported by a specialist GP with regular clinical supervision and will have the support of the wider multidisciplinary network and the Arch team. Movement between settings is often conducted on ebikes to facilitate case-finding.

This role is being advertised for 22.5 hours per week over three days (with flexibility)

Job responsibilities

Clinicalresponsibilities

• To lead the outreach team, reaching andproviding clinical care to people who are experiencing homelessness in variouscontexts around Brighton & Hove and who are struggling to access thesurgery; supporting in complex case management of patients with multiplecompound needs.
• To take clinical leadership of patient careacross various contexts, prioritising work effectively.
• To effectively engage patients from a variety ofbackgrounds.
• To independently undertake effectivecomprehensive holistic health assessments for patients including independentassessments of risk.
• To take time to listen to patients who may havehighly distressing personal circumstances and stories to understand theirneeds.
• To make appropriate referrals for patients asnecessary.
• To proactively identify the need for mentalcapacity assessments, safeguarding referrals, challenges to NHS chargingdecisions within the caseload and take action as required.
• To clinically advocate and negotiate on behalfof patients with partners, health colleagues and hospital staff.
• To attend case conferences andmulti-disciplinary team meetings as necessary.
• To work with the wider team and partners to provideopportunities for patients to engage with their health in the community orstreet.

Project Co-ordination

• To developsystems and processes to support the delivery of the project.
• To audit andprovide reports on the project when requested.
• To foster strongpartnerships with partner organisations such as CGL, SCFT, Justlife andothers.
• To manage staffassociated with this project, providing developmental leadership.
• To represent theoutreach team presenting at conferences and other local local and occasionalnational forums.
• Be willing toengage with teaching and training responsibilities for medical/nursing studentsgaining experience and/or support workers

Administrative

• To maintain robust systems of datacollection, audit and review of all clinical activities, liaising with fellowcolleagues in the practice and in the local and national health system asneeded to ensure good patient care and service is maintained and to satisfy thestandards of external regulatory bodies such as the CQC etc.
• To maintain accurate patient records,updating the practice system as is required for good patient care.

Communication

• Communicateeffectively with patients and carers, recognisingthe need for alternative methods ofcommunication to overcome different levels of understanding, culturalbackground and preferred ways of communicating.
• Utilise and demonstrate sensitivecommunication styles, to ensure patients are fully informed and consent totreatment.
• Utilise communication skills tosupport patients to adhere to prescribed treatment regimens.

Personal Responsibilities

• To maintain your own continuous CPDand ensure a PDP is kept up to date in line with the practice and nursing/clinicalteam development plans.
• Recognise and work within own competenceand professional code of conduct as regulated by your relevant professionalbody.
• To maintain your own annualregistration with the relevant professional body and adhere to its Code ofProfessional Conduct.
• Promote the Homeless Health Service ina positive way, adopting a flexible approach to problem solving and servicedelivery.

Person Specification

Knowledge/Skills

• Effective communication and interpersonal skills.
• Knowledge and understanding of current national developments and trends in health care and practice development.
• Experience of managing clinical risk and experience of identifying and mitigating external risks to outreach working including knowledge around organisational risk registers.
• Understanding of clinical coding in general and Read coding in particular.
• Competent and comfortable in completing a holistic needs assessments including physical health, mental health and substance use in patients experiencing homelessness.
• Implementing strategies for quality assurance and quality audit. Experience of innovative practice or working with new technologies in health care practice.
• Experience of TPP SystmOne clinical system.
• Experience of Microsoft Office software.
• Ability to ride an ebike.

• Registered Advanced Nurse Practitioner or Advanced Care Practitioner (prescriber).

• Full driving licence.
