An exciting opportunity has arisen with the introduction of a new Discharge Team. We are looking for an experienced, self‑motivated, and passionate person to join our team. The post holder will be based within the Hospital Flow structure, working across BHRUT Hospitals in the acute setting, and will be responsible for efficiently managing and overseeing discharges within inpatient wards, working closely with the Multidisciplinary Team (MDT), patients, families, our Integrated Discharge Hub, and community partners. Responsibilities

Provide senior oversight and support of complex discharges across different specialties. Manage a team of Discharge Coordinators, ensuring PRIDE values and inclusivity are demonstrated. Support the Trust’s strategic objectives of increasing hospital flow by achieving earlier discharges and reducing overall length of stay. Challenge factors that could delay discharges and ensure patients progress through the appropriate pathway. Collaborate with the Hospital Flow Team at Queen’s or King George Hospital and with Clinical Care Groups. Work with inpatient wards, patients, families, and community partners to progress discharge plans.

Qualifications

Proven experience in a similar discharge or patient flow role. Strong leadership skills with the ability to manage a small team. Excellent communication and stakeholder engagement abilities. Commitment to the NHS values and the Trust’s PRIDE values. Understanding of safeguarding responsibilities for children and adults.
