Summary: Working remotely, our team triages information from colleagues and members of the public in the Wolverhampton locality to ensure that we refer and signpost people to the most appropriate service to meet their needs. In summary our Care Coordination work streams include:

Responsibilities

  • Triage over the telephone or via video consultation to support people with an acute need to avoid admission; work closely with the Urgent Community Response Team and West Midlands Ambulance Service to achieve safe community care for acutely unwell Wolverhampton residents.
  • Triage over the telephone or via video consultation for patients with long-term conditions requiring ongoing health and/or therapy support while residing in their own homes.
  • Remote triage as the main role; offer a see-and-treat face-to-face triage service for patients with an unmet need who would be routinely referred to a District Nurse or Therapist, but would benefit from an assessment visit on the day of referral.
  • As a Junior Sister/Charge Nurse within the Care Coordination, provide managerial leadership support to the team, support staff in their development and lead with service innovation.

Qualifications

  • Experience and confidence in utilising computers and web-based applications is essential.
