Are you a Registered Dietitian passionate about helping people live healthier lives? Harmonia Collaborative Care is looking for aCertified Dietitian (Part-Time)to support individuals with nutrition services that directly impact their overall well-being—especially those with health-related social needs.

This is a flexible, part-time role perfect for someone who enjoys community-focused work and meaningful client engagement.

What You’ll Do:

• Deliver one-on-one and group nutrition counseling sessions
• Help clients with practical nutrition goals, including meal planning and grocery shopping guidance
• Collaborate closely with our Social Care Screener/Navigator to support coordinated care plans
• Assess dietary needs and design personalized nutrition interventions
• Ensure services align with NYS 1115 Program standards and compliance requirements
• Maintain accurate records and track client outcomes

What You Bring:

• Registered Dietitian (RD/RDN) or Registered Nurse Dietitian with CDN in NYS
• Bachelor’s or master’s in nutrition, Dietetics, or a related field
• 1+ year of experience in nutrition counseling or community health (Medicaid experience a plus!)
• Excellent communication skills and ability to work independently
