Job Description
Job Summary:
Reporting to the Client Services Supervisor, the Case Manager position is an integral part of a busy, dynamic and growing clinical program at Kinark Child and Family Services. The incumbent will be expected to exercise strong familiarity and knowledge of case management principles and practices and have awareness of community resources and services available within the children’s mental health sector. In addition, the Case Manager will be responsible for providing leadership and supporting the interdisciplinary clinical team in the coordination and provision of client intake, service delivery, discharge, and post-intervention, as required. The successful incumbent must be available to work evenings depending on client needs.
Kinark has introduced a benefit that gives employees in eligible roles the opportunity to work partially or fully remote if they choose to opt-in for this benefit.This role (Clinical Case Manager) is assessed as a Roamer position requiring 3 day in the office per week. This role is also eligible for the compressed work week program upon successful completion of probationary period.
Duties & Responsibilities:
- Connect and maintain collaborative relationships with community partners
- Advocate for clients internally and externally
- Liaise with child welfare, education, health and other community stakeholders and partners
- Manage pod wait list and support clients while waiting for service
- Conduct risk assessments and necessary safety plans and support clients on wait list
- Assign cases to pod therapists, expediting high risk clients into service
- Participate in program area admission to service process in collaboration with assigned therapist
- Prepare consents and facilitate the sharing of relevant collateral information
- Identify and facilitate referrals to internal resources (such as Psychiatry, Psychology, therapeutic groups), as well as to external resources (such as services provided by community partners)
- Coordinate and actively participate in client case conferences and treatment meetings (TPR/ATP)
- Complete necessary contact notes
- Attend regular supervision
- Attend interdisciplinary, pod and team meetings
Qualifications & Skills:
- A Master’s Degree in Social Work or a Health-Related Discipline with a minimum of 3 years relevant experience is preferred. **
- Candidates with a Bachelor’s Degree in Social Work with extensive and relevant experience will also be considered. **
- Must be registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) as a Social Worker, or the College of Registered Psychotherapists of Ontario (CRPO).
- Must be eligible and able to perform Psychotherapy as defined under the act.
- A minimum of 3 years relevant experience working in a mental health setting, is required. **
- Experience with providing a broad range of clinical services and demonstrated effective work with a child and adolescent population who present a variety of clinically challenging issues.
- Excellent clinical assessment, formulation and treatment planning skills is essential.
- Strong case management and crisis intervention skills.
- An understanding of the complexities of working with multi-stressed individuals and families is required.
- Experience and knowledge in crisis intervention is an asset.
- Strong organizational and time management skills.
- Strong Relationship Building, Engagement and Meeting Facilitation Skills.
- Travel is required for this position; the successful candidate must have a validOntario driver’s license, access to a reliable vehicle and be willing to obtain automobile insurance which includes driving for business purposes with a minimum liability of one million dollars.
- Satisfactory Criminal Record Check (including a Vulnerable Sector Screening) and Pre-Employment Medical.
- Candidates Bilingual in English and French will be given special consideration.
