Are you an experienced Clinical Lead Nurse (RNLD/RMN) who thrives in community settings and supported living? This role covers West London, managing a small, well-defined caseload of adults with complex needs across peoples own homes and supported living services. It's a full-time, permanent position, Monday-Friday, 9am-5pm with hybrid admin time (no shift work), offering £50,000-£55,000 per annum (DOE).

You'll provide clinical leadership and governance for complex care packages, ensuring safe, person-centred, and outcomes-focused support. Working closely with families, MDT partners, commissioners, and internal care teams, you'll raise standards, build capability, and champion best practice across LD/MH pathways.

Responsibilities

  • Hold clinical accountability for a small caseload across West London (community & supported living).
  • Complete holistic assessments; devise and review care plans, risk assessments, and clinical protocols (incl. MAR oversight).
  • Lead clinical governance activities: audit, incident review, lessons learned, and continuous improvement.
  • Coach, supervise, and upskill field teams (competency sign-off, training delivery, reflective practice).
  • Liaise with MDTs, ICBs/commissioners, and families; coordinate reviews and outcome tracking.
  • Lead on safeguarding (Adults/Children), MCA/DoLS documentation, and best-interest decisions.
  • Promote PBS and least-restrictive practice; escalate clinical concerns appropriately.

Requirements

  • RNLD or RMN with valid NMC registration.
  • Proven leadership in community complex care (LD and/or MH); supported living experience advantageous.
  • Strong clinical governance mindset (audit, documentation, CQC compliance).
  • Excellent communication, coaching, and stakeholder management skills.
  • Full UK driving licence; comfortable travelling across West London.
