Clinical Pharmacist - Harris Memorial Surgery

Overview

We are looking for an experienced pharmacist with an independent prescribing qualification to join our friendly general practice team.

You will work as part of our multi-disciplinary practice team, working closely with our pharmacist, doctors, nurses and dispensers to deliver high quality medicines management across the practice.

Responsibilities

  • Complete medication reviews including structured medication reviews for eligible patients;
  • Carry out medicines reconciliations for patients across care boundaries to ensure that our patients get the right medicines in a timely manner;
  • Answer prescription and medication queries from patients and staff;
  • Support safe prescribing of repeat medication including ensuring medication monitoring requirements are met;
  • Manage elements of the quality and outcomes framework and enhanced services such as Quality Improvement;
  • Proactively manage patients with complex polypharmacy;
  • Ensure the practice integrates with community and hospital pharmacy to help utilise the skills mix within teams, improve patient outcomes, ensure better access to healthcare and help manage workload.

About Us

Harris Memorial Surgery’s main site is in Illogan, near Portreath, and the branch surgery is in Lanner. We serve 6,500 registered patients across a large area across North Kerrier including Redruth and Camborne. We have a diverse population with some areas of high deprivation. We are always Open to consider new ways of working to innovate and improve care for our patients.
