Job Summary
The post holder will assist our service users in moving smoothly through the referral, treatment and discharge processes within the Tower Hamlets CAMHS service and to and from other services in their designated Borough. They will play a pivotal role in ensuring that families experience ease of entry to and a seamless transition between services, and that they feel supported to access the right service at the right time even when their needs are complex and may involve input from multiple teams.
Job Summary

Main duties of the job
This involves ensuring that service users in any stage of the care pathway are suitably risk assessed and, when not suitable, are referred to appropriate agencies that the post holder will liaise with, for example voluntary and education-based services. The post holder will also act as the main point of contact (in addition to the Clinician On Duty) for day-to-day risk issues by conducting triage assessments, risk calls, and liaison discussions with crisis services as appropriate. They will also support other clinicians in signposting and making onward referrals to other services. Playing a key role in enhancing inter-agency relationship and interface, the post holder will be supported, where appropriate, to attend team meetings and deliver collaborative projects/ interventions in identified settings as guided by the service's needs.

Essential
- Registered practitioner/Allied Health professional registered with a professional body, ideally HCPC: psychologist (BPS), clinical associate psychologist (BPS), senior wellbeing practitioner/PWP/CWP/EMHP (BPS/BABCP), CBT therapist (BABCP), counsellor, psychotherapist (BACP), social worker, family therapist, occupational therapist, speech and language therapist, RMN/RNLD nurse
- Post graduate qualification in mental health
Experience, Skills and Abilities
- Significant experience providing assessment and intervention with children, young people and families
- Experience of working as part of a multi-disciplinary team
- Experience of screening and managing referrals
- Experience of using outcome measures
- Experience of completing risk assessment and management plans
- Ability to meet agreed/specified service targets
- Ability to manage own caseload and time
- Ability to utilise clinical supervision and line management
- Excellent verbal and written communication skills. Demonstrate highstandard in written communication e.g. letter to referrers
