Head of Integrated Care – Primary Care Networks
Location: East of England
Rate: £600–£650 per day
Status: Inside IR35
Contract: 6-month contract initially, likely to extend
Working pattern: Hybrid working – 2-3 days per week on site (flexible)

We’re working in partnership with a forward-thinking local authority and NHS community provider to recruit a Head of Integrated Care – Primary Care Networks. This senior leadership role provides operational and strategic oversight of integrated health and social care services across multiple localities.
This is an exciting opportunity to drive the integration of health and social care, improve service delivery, and support the development of Primary Care Networks to deliver seamless, person-centred care across communities.
Your role as Head of Integrated Care:
- Provide operational and strategic leadership across health and social care functions.
- Develop and embed integrated working across local authorities, NHS partners, and primary care networks.
- Lead service development, performance improvement, and workforce planning initiatives.
- Oversee budgets, staffing, and resource allocation to ensure cost-effective service delivery.
- Implement national and local health and social care initiatives, ensuring compliance and best practice.
- Build strong, collaborative relationships with commissioners, GPs, acute providers, and voluntary sector partners.
- Promote patient and service user engagement in shaping and improving local services.
Your experience as a Head of Integrated Care:
- Senior management experience in health or social care within a complex, multi-agency environment.
- Strong track record of leading integrated service transformation and performance improvement.
- Proven experience managing budgets, staffing, and operational delivery at scale.
- Excellent understanding of community health, social care pathways, and Primary Care Network models.
- Skilled communicator and influencer, able to build trust and drive collaboration across partner organisations.
- Relevant degree and ideally a postgraduate qualification in health, social care, or management.
