Overview
If you're a Mental Health Nurse, Occupational Therapist, Social Worker or other registered professional working within Mental Health and want to work with and across a variety of GP surgeries in the City of Norwich, this could be the ideal role for you!
As Lead PCN Mental Health Practitioner, you will transform community mental health services by working with the Integrated Care System to offer Place Based Care, ensuring communities receive fast and effective treatment in the familiar surroundings of their GP surgery.
You will shape the service while managing a small team of Mental Health Practitioners. You will set up and deliver patient clinics that provide support, medication advice (if qualified to prescribe), treatment, education and problem-solving approaches for community members experiencing mental health challenges. You will provide a dedicated growing service that meets individual needs in a seamless environment. We are happy to accept full time or part time applications and can discuss flexible working patterns within GP practice hours.
Main duties of the job include working within Primary Care Networks (PCNs) in Norwich, leading a small team, managing day-to-day clinical issues, running autonomous clinics, and providing an intervention-led mental health service for people who need specialist care not available through NHS Talking Therapies or Secondary Care. You will work with patients aged 16 and above and act as a conduit into specialist mental health services when required.

- Lead a small team of Mental Health Practitioners and manage day-to-day clinical and operational issues in allocated GP practices.
- Set up and run patient clinics offering support, treatment and education for mental health needs.
- Provide intervention-led care for patients aged 16 and above; refer to specialist services when required.
- Collaborate across GP practices within Norwich PCNs to bridge primary and secondary care.
