Learning Disabilities Assessment Manager at Liaison Group. Liaison Care partners with ICBs and Local Authorities across England and Wales to deliver high-quality, innovative support. We are seeking an experienced Learning Disabilities Assessment Manager to support growth and leadership in this area.

Location: Liaison Court, Vincent Road, Worcester, Worcestershire, WR5 1BW. Salary: £60,000 to £75,000 a year. Contract: Permanent. Working pattern: Full-time, Home or remote working. Date posted: 02 September 2025.

Responsibilities

  • Conduct Comprehensive Assessments: gather and review documentation prior to assessment (including medical records and care plans); undertake clinical assessments involving individuals, their representatives, care providers, and Local Authority professionals.
  • Evaluate Needs and Eligibility: assess ongoing health and social care needs (medication, daily living activities, provider input); determine eligibility for Continuing Healthcare (CHC) funding and delineate between health and social care responsibilities; make evidence-based recommendations incorporating Local Authority input.
  • Provide Clinical Expertise: offer clinical advice throughout assessment, care planning, and review; ensure care packages meet assessed clinical needs with appropriate support levels.
  • Case Review and Monitoring: regularly review cases to confirm continued eligibility and alignment with clinical need; develop and apply KPIs to monitor care package effectiveness.
  • Care Planning and Documentation: complete assessment documentation accurately and timely; update care plans reflecting assessment outcomes and Local Authority input; draft clear recommendations for submission to the Integrated Care Board (ICB) with robust evidence.
  • Support Individuals and Families: provide information and guidance during the assessment process; respond to complaints with detailed evidence and a person-centred approach.
  • Collaborative Working: work with multidisciplinary teams including healthcare providers, local authorities, and commissioning colleagues to ensure joined-up care planning and delivery.

Qualifications

  • Essential: Degree-level qualification; Experience in Learning Disabilities CHC assessments; experience redesigning complex care packages to meet clinical and social care needs; self-motivation; ability to manage complex assessments; strong relationship-building with partner organizations; experience of working with individuals and families.
  • Desirable: Experience of remote assessments.

Person Specification

  • Essential: Qualified to degree level.

Other requirements

  • Disclosure and Barring Service (DBS) check: post subject to Rehabilitation of Offenders Act provisions; submission for DBS disclosure as applicable.
  • UK Registration: current UK professional registration required.
