Job Description:
We are seeking a proactive and compassionate Learning Disabilities Navigator to join our team. In partnership with the health and social care teams, individuals, carers, family and circles of support including voluntary sector, to provide identified people who have a learning disability with time-limited, supported sign-posting service, coordinating the support offered within the community from the voluntary and statutory sectors and ensuring they experience ‘joined-up’ services.
The role will involve working with individuals with LD, helping them to stay connected, informed, and resilient. A key part of the role is promoting local Live Well Hubs and gathering feedback to support service development.
All Age Navigators will support people to obtain community solutions reducing the demand on statutory services and promote the education and empowerment of self-help. They will build and enhance the existing strong relationships with GP surgeries and hospital in Telford. They will promote and support people to access Live Well Hubs across the borough.
Main Duties and Responsibilities:

Act as a first point of contact for individuals and families, providing support and signposting to early help and prevention services including the use of a new pathway into Citizens Advice for fast-track appointments offered to those with LD
•
Encourage and support attendance at local Live Well Hubs and relevant community activities.
•
Build strong relationships with individuals, families, and partner organisations to ensure joined-up support.
•
Monitor and report on engagement, outcomes, and barriers to access, feeding back insights to inform service planning and delivery.
•
Maintain accurate records of interactions, referrals, and outcomes in line with data and reporting requirements.
•
Help to shape the future of LD support by identifying gaps and sharing real-world insights from the community this will include the attendance at the LD Partnership Board (Quarterly)

It is expected that the successfulcandidate will carry out other reasonable duties from time to time as appropriate.
The post holder will be based predominantly at the Wellbeing Cafe in Madeley and at a new Live Well Hub (Location to be confirmed but likely Wellington).
The service operates a friendly, informal team. However, we aim to maintain the highest standards of service and efficiency. Experience of working in a small team in a busy office environment would, therefore, be an advantage.
Support, supervision and training, where necessary, will be provided.
Qualifications / Experience
Evidence of a solid general education is required together with GCSE A*-C / 9-4 or equivalent including English and Mathematics along with good keyboard skills and the ability to use Microsoft Office applications, i.e. Word, Excel, Outlook, Internet Explorer and an understanding or knowledge of Access.

Experience of working with LD individuals across all ages.

Strong communication and interpersonal skills.
•
Confident working independently in community settings.
•
Knowledge of local services and support networks.
•
Ability to record, track and report outcomes in a clear and meaningful way.
Evidence of further education or training that would support your application.
Hours of Work: Normal office hours are Monday to Friday 9.00 am – 5.00 pm. “Out of Hours” work and a flexible working pattern will be necessary.
The successful candidate will be expected to normally work Monday & Thursday
Time off in lieu can be arranged (overtime is not paid).
