Main duties of the job

As the Neighbourhood Health Programme Manager, you will lead the design and delivery of integrated neighbourhood health services across Merton. You'll bring together teams from primary care, community services, social care, and the voluntary sector to create joined-up models of care that focus on prevention and personalisation. Your day-to-day work will involve coordinating partners, embedding data-led decision-making, developing the local health workforce, and ensuring the voices of local communities shape services. Salary Band: Indicative £64,455 to £74,896 depending on experience

Role Purpose:

The NHP Programme Manager will lead the development, delivery, and ongoing improvement of Integrated Neighbourhood Teams (INTs) through leading and managing the programme delivery of the Merton Neighbourhood Health Working Group (MNHWG) working closely with its co-chairs. They are also expected to attend the East and West Neighbourhood Locality Delivery Groups.

The postholder will ensure that general practice is at the heart of proactive, personalised, and preventative neighbourhood health models.

Key Responsibilities:

Primary Care Leadership in Neighbourhood Health

• Champion general practice leadership in INTs and neighbourhood structures.
• Strengthen resilience and sustainability of general practice through coordinated workforce, infrastructure, and clinical pathways.
• Ensure practices are central to integrated working, with active MDT participation.

Operational Neighbourhood Delivery

• Deliver "Team of Teams" models coordinating primary care, secondary care, community services, mental health, adult social care, public health, housing, and VCFSE partners.
• Implement neighbourhood-based anticipatory care, proactive case finding, and prevention programmes.
• Lead local implementation of Core20PLUS5 priorities to reduce health inequalities.

Population Health Management

• Embed PHM methodologies to segment populations, identify risk, and co-design interventions working with BI teams across sectors and SWL ICS.
• Champion use of population health data to drive strategic decision-making and operational delivery.

Access and Digital Transformation

• Support delivery of 24/7 digital and telephonic "neighbourhood gateways".
• Champion the adoption of integrated digital tools for real-time patient information sharing.
• Promote digital-first approaches while ensuring inclusivity.

Workforce and Organisational Development

• Develop and support a multidisciplinary workforce including ARRS roles.
• Foster a culture of quality improvement, learning, and resilience.
• Lead OD initiatives that embed strengths-based, population-focused, and prevention-led team cultures.

Governance and Sustainability

• Support the establishment and management of neighbourhood governance and risk management structures.
• Monitor outcomes against local, system, and national frameworks.
• Lead local strategies to enhance resilience and stability in primary care and community services.

Partnership and Community Engagement

• Build effective, trusted relationships across health, social care, public health, VCFSE, and housing sectors.
• Lead engagement with local patients, carers, and communities, ensuring co-production and shared decision-making.

Finance and Resource Management

• Manage agreed delegated budgets where applicable, ensuring efficient use of resources aligned to population health needs in relation to the work of the neighbourhood health working group and locality neighbourhood health delivery groups.
• Work with identified integrator organisations to align investment with neighbourhood delivery priorities.

Person Specification

Qualifications:

• Degree-level education or equivalent experience in health, social care, or management.
• Evidence of continued professional development.

Experience:

• Significant leadership experience within general practice, PCNs, integrated community care, or system transformation roles.
• Demonstrable success in leading integrated, multi-agency, or multidisciplinary teams.
• Proven experience in population health management and health inequalities programmes.
• Experience of managing change, service improvement, and workforce transformation.
