Main duties of the job

As the Neighbourhood Health Programme Manager, you will lead the design and delivery of integrated neighbourhood health services across Merton. You'll bring together teams from primary care, community services, social care, and the voluntary sector to create joined-up models of care that focus on prevention and personalisation. Your day-to-day work will involve coordinating partners, embedding data-led decision-making, developing the local health workforce, and ensuring the voices of local communities shape services. This is a leadership role at the forefront of transforming how care is delivered at a local level.

Job responsibilities

Salary Band: Indicative £64,455 to £74,896 depending on experience

Accountable to: Chief Executive Officer

Reporting to: Co-Chairs of the Neighbourhood Health Working Group

Location: Merton; Base: Merton Civic Centre

Role Purpose:

The NHP Programme Manager will lead the development, delivery, and ongoing improvement of Integrated Neighbourhood Teams (INTs) through leading and managing the programme delivery of the Merton Neighbourhood Health Working Group (MNHWG) working closely with its co-chairs. They are also expected to attend the East and West Neighbourhood Locality Delivery Groups.

The postholder will ensure that general practice is at the heart of proactive, personalised, and preventative neighbourhood health models.
