Were looking for a highly motivated, passionate Registered Nurseto join our expanding Hospice at Home (H@H) Team at St. Michaels Hospice. Thisis a specialist nursing role, providing compassionate, expert care to patientsin the last weeks and days of their lives. This is an ideal opportunity for a nurse who wishes to specialisein palliative and end of life care, or who ideally already has experience inthis area. Someone who has a passion for making a difference to patients andtheir families alongside a love of the nursing role are some of the keyaspirations we are looking for in the successful candidate. Experience ofworking within the community setting would be an advantage.

The successful candidate will be anNMC-registered nurse with an active PIN. You will have excellent communicationskills and empathy. Youll work both independently and as part of a supportivemulti-disciplinary team, proactively assessing patients needs, planning anddelivering high-quality care, and advocating for the best possible outcomes.

This role involves predominantly working night shifts(19:30 20:00), over a 7 day week, with the flexibility to work day shifts(07:30 20:00). A full UK driving licence and access to a vehicle areessential, as you will be travelling across the community within the hospicescatchment area

Main duties of the job

We are launching a new 24/7 Hospice at Home service and are recruiting to new positions within this service. The 24/7 service will offer a visiting service to patients within our catchment known to be in the last 2 weeks of life.

Working collaboratively with the wider multi-disciplinary team, youll assess, plan, and provide holistic care that meets the physical, emotional, and spiritual needs of patients. Youll also play a key role in contributing to service improvement and promoting best practice in end-of-life care.

Job responsibilities

The Hospice at Home multi-professional Team(H@H) provide a 24/7 planned and reactive service to those deemed to be intheir last 2 weeks of life who require specialist palliative care advice andsupport.

The H@H nursing team specialise in providingpalliative care symptom advice, holistic assessments and support to patients atthe End of Life. The team provide clinical nursing interventions and support toenable the patient to remain at home, prevent hospital admissions and supportdischarge from hospital to home.

In this role, you will be responsible forassessing patients care needs and for developing, implementing, and evaluatingpersonalised care plans. This will be done in partnership with patients,families, and other professionals to ensure both physical and psychologicalneeds are met.

The H@H team also support patients activelyunder the care of St. Michaels Services to provide a community patientvisiting service which includes patients known to the SMH Living Well Services,SMH MND Service and the SMH Long-Term Neurological Conditions Service.

The H@H nursing team is supported by thewider multi-professional Hospice at Home team at St. Michaels Hospice.
