Here in the Discharge to Assess Pathway 1 Team we have fantastic opportunities for therapists looking for the next step in their career. We have 4 small friendly teams across Lincolnshire comprising of Occupational Therapists, Physiotherapists, Nurses, Pharmacy Technicians, Promoting Independence Therapy Assistants and Promoting Independence Care Assistants. We are a growing team and looking for enthusiastic, friendly, and proactive therapists.

Responsibilities

  • Work in the Pathway 1 Team, following the patient home and seeing them within 2 hours of discharge.
  • Listen to the patient to understand what matters most to them in relation to personal and social circumstances, physical and mental health, and carer needs.
  • Provide exercise programmes for physical and mental wellbeing; recommend and supply the correct equipment; promote independent living.
  • Educate patients on self-management of health and wellbeing, understand frailty symptoms, and reinforce falls prevention and overall wellbeing.
  • Collaborate with colleagues across sectors to support patients in the community and maintain a high standard of therapy provision.
  • Contribute to the Quality Service Improvement Programme and bring creative, resourceful, and innovative ideas with personal ownership of projects.
  • Grow within the team and develop in line with Lincolnshire Community Health Services (LCHS) values and professional development.
