This is an exciting opportunityfor two Advanced Nurse Practitioners to join South Lincoln Healthcare PCN aspart of our expanding clinical team.

The post holder will work withinand across our 7 member practices (Bassingham Surgery, Branston &Heighington Family Practice, Brant Road Surgery, Church Walk Surgery, CliffVillages Medical Practice, The Heath Surgery, Washingborough Family Practice)along with undertaking work for the PCN.It is anticipated that the post holder will mainly work across 3-4practices, with a need to cover other practices on occasion, depending on need.

The role will support the PCNspassion for providing an all-encompassing approach to personalised care andhealth inequalities across the PCN.

Part time/job share would be considered.

The post holder will need to hold a full driving licence and be prepared to travel between locations to support the remit of the role.

Main duties of the job

The post holder(s) will be anintegral part of both the PCN and General Practice Teams, and will workalongside GPs, Practice Nurses, HCAs, GPAs, Clinical Pharmacists, PharmacyTechnicians, Occupational Therapists, Social Prescribing Link Workers, Health& Wellbeing Coaches, Care Coordinators, Mental Health Practitioners and MusculoskeletalPractitioners.

The role will mainly consist of workingwithin our member practices to enhance and support day to day access andcapacity by providing a mixture of on the day and routine consultations whichmay be face to face, telephone, online or home visits. Time will be allocated for associated administration.

In addition, the postholder(s)will spend a smaller proportion of their time supporting the PCN by workingwith our care home team. This would involve supporting with triage, telephoneconsultations and visits to care homes.

For a suitable candidate, therewill be the opportunity to provide programme leadership for of our Cancerand/or Cardiovascular Disease Programmes and leadership for nurses (eg theimportance of SMI health checks, elderly health checks, and LD health checks).

The post holder will have accessto skills development and support so they are able to further build theirskills and experience within the role.

The role willrequire the ability to work flexibly to meet the needs of the organisation andthis may include working outside of core General Practice hours or within ourEnhanced Access service.

Job responsibilities

• Integrateand work within our member practices to enhance and support day to day accessand capacity.
• Provideadditional capacity support for the PCN care home team.
• Assessand triage patients, including same day triage, and as appropriate providedefinitive treatment (including prescribing medications following policy,patient group directives, NICE (national) and local clinical guidelines andlocal care pathways or make necessary referrals to other members of the PrimaryCare team.
• Manageundifferentiated undiagnosed conditions and identify red flags and underlyingserious pathology and take appropriate action.
• Use complex decision making to inform thediagnosis, investigation, complete management of episodes of care within abroad scope of practice.
• Activelytake a personalised care approach and population centred care approach toenable shared decision making with the presenting person.
• Completethe relevant training in order to provide multi-professional, clinical practiceand CPD supervision to other roles within Primary Care, for example, firstcontact practitioners and personalised care roles.
• Undertakeclinical audit and quality improvement work across the PCN.

Person Specification

Experience

• Have at least 1 years advanced clinical practice experience in a primary care or urgent care setting to evidence autonomous clinical practice
• Be working at masters level or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competencies
• Highly developed specialist knowledge which is underpinned by theory and experience
• Experience of dealing with vulnerable patients
• Has a personalised care and What Matters to Me approach embedded within working practices
• Ability to work under pressure and manage varied workloads with changing demands
• Ability to see patients of all ages, including children
• Experience of working in a multi-disciplinary team within a primary care environment
• Knowledge of local primary care services

• NMC registered non-medical prescriber
• One of the following must apply to Masters level study:
• -Graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (Supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre for Advancing Practice, demonstrating recognised educational and experiential preparation in advanced practice
• -Currently enrolled as a trainee on a Centre for Advancing Practice accredited MSc advanced practice programme
• -Currently enrolled on and progressed past the initial Learning Needs Analysis (LNA) stage of the Centre for Advancing Practice ePortfolio (supported) Route
• Evidence of continued professional development
