PCN Advanced Nurse Practitioner

This is an exciting opportunityfor two Advanced Nurse Practitioners to join South Lincoln Healthcare PCN aspart of our expanding clinical team.

The post holder will work withinand across our 7 member practices (Bassingham Surgery, Branston &Heighington Family Practice, Brant Road Surgery, Church Walk Surgery, CliffVillages Medical Practice, The Heath Surgery, Washingborough Family Practice)along with undertaking work for the PCN.It is anticipated that the post holder will mainly work across 3-4practices, with a need to cover other practices on occasion, depending on need.

The role will support the PCNspassion for providing an all-encompassing approach to personalised care andhealth inequalities across the PCN.

Part time/job share would be considered.

The post holder will need to hold a full driving licence and be prepared to travel between locations to support the remit of the role.

Main duties of the job

The post holder(s) will be anintegral part of both the PCN and General Practice Teams, and will workalongside GPs, Practice Nurses, HCAs, GPAs, Clinical Pharmacists, PharmacyTechnicians, Occupational Therapists, Social Prescribing Link Workers, Health& Wellbeing Coaches, Care Coordinators, Mental Health Practitioners and MusculoskeletalPractitioners.

The role will mainly consist of workingwithin our member practices to enhance and support day to day access andcapacity by providing a mixture of on the day and routine consultations whichmay be face to face, telephone, online or home visits. Time will be allocated for associated administration.

In addition, the postholder(s)will spend a smaller proportion of their time supporting the PCN by workingwith our care home team. This would involve supporting with triage, telephoneconsultations and visits to care homes.

For a suitable candidate, therewill be the opportunity to provide programme leadership for of our Cancerand/or Cardiovascular Disease Programmes and leadership for nurses (eg theimportance of SMI health checks, elderly health checks, and LD health checks).

The post holder will have accessto skills development and support so they are able to further build theirskills and experience within the role.

The role willrequire the ability to work flexibly to meet the needs of the organisation andthis may include working outside of core General Practice hours or within ourEnhanced Access service.
