We have FULL TIME REMOTE vacancies for Prescribing Pharmacists and Non-Prescribing Clinical Pharmacists, including PCN Pharmacists. The post holder will be an experienced Clinical Pharmacist, PCN Pharmacist or Practice Pharmacist who is enthusiastic about making a tangible difference to patients’ lives. Applicants MUST have previous general practice experience or prior experience working as a GP Pharmacist. Core Prescribing Solutions is an award-winning, dynamic, fast-paced healthcare organisation with over 25 years of combined NHS, primary care, health informatics and pharmacy sector experience. We provide tailor-made, tech-enabled primary care services through a fully managed clinical pharmacist or technician-led model for PCNs, GP practices and GP federations to help reduce workloads and streamline processes.

Main duties

  • Independent prescribing qualification for prescribing pharmacist (not essential for non-prescribing roles)
  • Qualification or enrolment on CPPE Clinical Pharmacist Training pathway (offered for PCN pharmacists to enhance clinical acumen)
  • Strong clinical leadership, communication and interpersonal skills; good understanding of PCN Pharmacist working
  • Contributing to the improvement of health outcomes in long-term conditions
  • Providing specialist knowledge and advice on pharmaceutical matters
  • Reviewing patients’ acute medicine requests
  • Completing discharge and clinic letters
  • Drawing up structured medication reviews as PCN Pharmacist
  • Advising and supporting patients with self-limiting conditions
  • Delivering excellent clinical care as a practicing Pharmacist with prior Clinical/PCN/Practice Pharmacist experience
  • Supporting practice achievement of key IIF/PCN Pharmacist and QOF targets as required
