The role of the PCN Project Support Officer will support various services and contractual requirements run within the PCN, including specific project work, care coordination and standing administrative responsibilities. They will also assist the PCN Management Team in maintaining robust data collection and reporting processes and ensure compliance with relevant procedures and policies.Please note that this job requires travel between all 5 PCN practices and working at multiple sites.Interviews will take place w/c 13th October 2025Main duties of the jobProjectSupportProvideproject support for existing and future projects of LCS PCNSupportthe Enhanced Access ServiceDevelopand run searches on SystmOne clinical systemUndertakedata analysis and assist with a range of services evaluation monitoringEngagewith relevant clinicians for the services and stakeholder practices of the PCNSupportthe proactive identification and management of patient caseloads in line withthe PCN DESWorkingcollaboratively with colleagues across the PCN, Health, Social Care and VCSEsectors providing patient centred, integrated care.Assistthe PCN Management Team with collation and production of project and servicedocumentation.Developand maintain databases and systems for monitoring and evaluationEngagewith the PCN practices to present findings, recommendations and to deliveractions agreedSupporttraining of staff as appropriate to improve service outcomesAdministrationTasksSupportthe administrative tasks of services and PCN DES contract delivery thatincludes appointment and referral management for patientsProvideadministrative support to the PCN Board, including agenda setting, roombooking, informing people of their follow-up action pointsSupportthe PCN Management Team in the day to day running of the PCN, ensuringcompliance with relevant policies and proceduresResponsiblefor developing and maintaining systems for storage and retrieval ofinformation, ensuring information is accessible to other members of the PCNAbout usWe are a 5-practice PCN working closely with our colleagues in community, mental health and social care, to improve the health and wellbeing of our PCN population.
