Job summary

Farnham Medical Centre have a fantastic opportunity for an experienced practice nurse to join our team, 33 hours per week (can be flexible for the right candidate). We are a large practice with 17k patients. We have 6 supportive GP partners, 3 salaried GPs, and a team of Advanced Nurse Practitioners, Practice Nurses, Treatment room nurse, Minor Illness nurse, Nurse Associate and Health Care Assistants, who work alongside a large admin team.

Main duties of the job

Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
Managing and treating long-term conditions, in line with national and local policies and the practice needs.
Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
Meeting the needs of patients presenting for opportunistic wound care.
Hypertension review and management
Running asthma, COPD and diabetes clinics and reviews

Farnham Medical Centre is a large GP practice in South Shields with over 17k patients. We are a busy practice, with teams of Doctors, Advanced Nurse Practitioners, Minor Illness Nurse, Practice Nurses and Health Care Assistants, who are supported to provide excellent patient care by a team of Managers, Care Navigators, Administration Staff, GP Assistants, and Secretaries.

To be instrumental in the support of the Infection Prevention Control (IPC) lead and to work to the IPC requirements by providing audits, training, support and evidence of compliance to the practice manager.
To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice.
To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
To develop, implement and embed health promotion and wellbeing programmes
To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health.
As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF.
As required, to diagnose and manage acute and chronic conditions.
Provide wound care (ulcer/Doppler, etc.) to patients.
To review patient medications to enhance compliance.
To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages depending on training/competence
To co-ordinate the provision of travel medicine services.
To request pathology services and process pathology results as required.
To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required, including visiting patients in their own homes
To maintain accurate clinical records in conjunction with extant legislation.
To ensure SNOMED CT codes are used effectively by all members of the nursing team.
To ensure the appropriate maintenance and use of chronic disease registers.
