Remote Primary Care Clinical Pharmacist-IP (TPN-REM-CPIP)

Job Responsibilities:

• Support and work alongside a team of pharmacists and GPs, acting within professional boundaries.
• Resolve medication-related queries and conduct medication reviews, including polypharmacy and SMRs.
• Perform telephone general medication reviews to improve patient outcomes and access to healthcare.
• Assist practice staff with prescription and medication queries.
• Conduct medicine reconciliation and review post-discharge.
• Carry out medical-related audits and contribute to service improvement initiatives.
• Provide expert advice on medications to clinicians and patients.
• Deliver clinical leadership on medicines optimisation and quality improvement.
• Ensure the appropriate and efficient use of medication.
• Educate patients on medication use and disease prevention.
• Understand frameworks such as QOF, CQC, and local incentive schemes.
• Create and follow a clinical pharmacy programme aligned with practice policies.
• Maintain excellent communication and interpersonal skills.
• Conduct reviews for patients with long-term conditions.
