Senior Nurse Assessor Children and Young People

The Senior Nurse Assessor plays a vital role in improving the lives and outcomes of Children and Young People (CYP) in Kent and Medway living with physical and/or mental health conditions.Working within the Children and Young People\'s (CYP) Continuing Care Service, the post holder will assess, review, and monitor care packages, and placements for CYP referred to All Age Continuing Care (AACC) and Specialist Placements.Commissioned by the Kent and Medway Integrated Care Board (ICB), this role supports the delivery of high-quality, evidence-based services that minimises the impact of illness and promote holistic well-being across physical, mental, and emotional health domains.

Main duties of the job

You will conduct evidence-based assessments to determine eligibility for NHS Continuing Care, presenting findings to a multidisciplinary panel. They must demonstrate strong clinical assessment and analytical skills and be confident in making clinical decisions. Upon eligibility confirmation, they will coordinate care packages and conduct regular reviews, with reassessments annually or as needs change.The role involves collaboration with a wide range of services, including local authorities, education and social care teams, children\'s health and disability services, hospitals, hospices, and adult Continuing Health Care.Travel across Kent and Medway, including out-of-area placements and hospitals, is required.Flexible working is supported through multiple office locations and a hybrid model of office and home-based work. The post holder will also provide clinical support to team members.

Person Specification

Qualifications

• Registered Clinician with current registration (Learning Disability, Children\'s Nursing, NMC)
• Evidence of relevant post registration training

Experience

• Working with Children and Young People with complex health needs in a variety of settings
• Experience of managing risks and reporting
• Assessment and Case management

Attributes

• Ability to analyse very complex issues where material is conflicting and drawn from multiple sources
• Able to both lone work & work in partnership
• Comprehensive knowledge of interventions CYP specific
• Ability to clearly present assessments and articulate the clinical reasoning behind decisions when asked or challenged
• Demonstrate capabilities to manage your own workload and make informed decisions in the absence of required information, working to tight and often changing timescales
• Managing CYP Transitions
