The post is based at the Royal London Hospital and requires working jointly with hospital and community teams across the North East London (NEL) Network to enable a smooth transition to adult services. The role involves designing transition pathways, health promotion, and impacting health outcomes for children and young people (CYP) as they move to adult care.ResponsibilitiesThe post holder will advocate for children and young people transitioning to adult care in Barts Health and across East London and Essex networks. This includes working closely with the multidisciplinary team to provide support, advice and education for teenagers and young adults in inpatient and outpatient settings up to age 25.Work across paediatric and adult services within the trust.Act as a resource for teaching and educating patients and staff.Develop CYP strategic priorities and contribute to service development using improvement methodologies.QualificationsEssentialRN children’s / RN adultPreparation for mentorship course or equivalentEvidence of continued professional developmentDesirableEducated to degree level or working towards itExperienceEssentialMinimum of 2 years’ experience at band 5 or above within a related clinical specialist environmentDesirableHaematology courseSkillsExcellent verbal and written communication skillsExcellent interpersonal skillsOrganisational skills; ability to prioritise and meet deadlinesAbility to provide nursing leadership and advice within the service and to lead and motivate staffFlexibility in working practices to meet service user needs and changes in service modelsAbility to work creatively with practitioners from other disciplines and agenciesAbility to plan, manage and implement service developments using improvement methodologiesAbility to facilitate reflective practice and carry out tasks with limited supervisionIT skills and ability to maintain accurate data #J-18808-Ljbffr
