The post is based at the Royal London Hospital and requires working jointly with hospital and community teams across the North East London (NEL) Network to enable a smooth transition to adult services. The role involves designing transition pathways, health promotion, and impacting health outcomes for children and young people (CYP) as they move to adult care.ResponsibilitiesThe post holder will advocate for children and young people transitioning to adult care in Barts Health and across East London and Essex networks.
