Job Summary

As a Clinical Pharmacist, you will work as part of a multidisciplinary team to provide expert medication reviews, support long-term condition management, and contribute to safer, more effective prescribing across our practices. You will play a pivotal role in improving patient outcomes through structured medication reviews, managing repeat prescribing, and supporting clinicians with medicines optimisation.

Responsibilities

  • Conduct structured medication reviews with patients to optimise treatment and reduce risk.
  • Support management of long-term conditions through evidence-based prescribing.
  • Provide expert advice on medicines to clinicians and patients.
  • Assist in medicines reconciliation post-hospital discharge.
  • Contribute to prescribing audits, safety initiatives, and formulary adherence.
  • Work closely with GPs and the wider clinical team to enhance prescribing quality.

About Us

We are expanding our pharmacy team and are recruiting for two Clinical Pharmacists and one Pharmacy Technician. Fully based in Devon with primary sites in Plymouth or Barnstaple, roles offer on-site and remote work to support our network of practices across the region. These are full-time roles (up to 37.5 hours per week); flexible or part-time working arrangements can be discussed to support work-life balance. You will deliver high-quality medicines optimisation, support long-term condition management, and contribute to safe, effective prescribing within our primary care network (PCN).

Job Description

Medication Management: Regular medication reviews to optimise therapeutic outcomes and minimise adverse effects, with adjustments in consultation with the Senior Clinical Pharmacist and other healthcare providers. Ensure prescription requests are handled appropriately. Monitor for drug interactions and contraindications. Resolve medication issues from hospital communications. Complete medicines-management tasks daily.

Patient Care and Counselling: Provide comprehensive counselling on medication use, side effects, interactions, and storage. Support informed clinical decisions with up-to-date data and guidelines. Participate in chronic disease management and care planning.

Collaboration and Communication: Work with doctors, nurses, and other healthcare professionals to ensure coordinated patient care. Provide drug information to the healthcare team and engage with patients to gather information on health status and treatment responses.

Person Specification

Essential

  • MPharm degree or equivalent; registration with the General Pharmaceutical Council (GPhC) or equivalent is mandatory.
  • Strong clinical pharmacology knowledge; proficiency in using Electronic Health Records (EHR) and other pharmaceutical software.
  • Thorough understanding of regulatory standards (GDPR, medication safety, clinical governance) and excellent communication skills.

  • Analytical and problem-solving skills; ability to monitor and evaluate treatment outcomes.
  • Strong interpersonal skills and willingness to engage in professional development and research.
