We are looking for a Nurse Practitioner to work between 24 - 37.5 hours per week.

Job responsibilities
We are seeking a motivated and compassionate Nurse Practitioner to join our team. The ideal candidate will be responsible for providing comprehensive care to our patients, working collaboratively with our multidisciplinary team.
Key Responsibilities
- Practice autonomously within scope using a person-centred, partnership-based approach.
- Assess and diagnose individuals accurately during acute and long-term health episodes.
- Gather and evaluate clinical data to make informed, evidence-based decisions. Carrying out referrals if necessary.
- Coordinate and manage care plans to optimise health outcomes and resource use.
- Provide emotional support and reassurance to individuals, carers, and families.
- Order, carry out and interpret tests such as blood work, imaging, or screenings to support diagnosis and treatment planning.
- Apply sound clinical judgment and use up-to-date research to guide decisions and care.
- Deliver health promotion and disease prevention advice, including support for smoking cessation, weight management, and contraception.
- Manage long-term conditions by providing patient education.
- Support minor illness and injury management, including prescribing and treatment planning within scope of practice.
- Use appropriate screening tools to confirm diagnoses and guide treatment.
- Contribute to multidisciplinary team meetings and collaborate with GPs, pharmacists, and allied health professionals to ensure coordinated care.
- Maintain accurate, timely clinical documentation and contribute to audit, quality improvement, and service development initiatives.
- Maintain accurate and up-to-date patient records in line with legal, professional, and practice standards.
- Manage repeat prescriptions and ensure safe prescribing practices are followed.
- Contribute to the development and review of clinical protocols, policies, and procedures.
- Participate in clinical audits, significant event reviews, and quality improvement initiatives.
- Support the induction, supervision, or mentoring of new staff, students, or trainees.
- Attend and contribute to practice meetings, multidisciplinary team discussions, and service planning.
- Ensure compliance with safeguarding, infection control, and health and safety policies.
Person Specification
Qualifications
- Registered with the Nursing and Midwifery Council (NMC) to practice as a Registered Nurse
- Completed an NMC-approved nursing degree (BSc or Nursing Degree Apprenticeship)
- Evidence of a commitment to on-going personal & professional development
- Postgraduate Diploma or Certificate in a Specialized Area
- Teaching and Mentorship Qualification
Experience
- Experience working as a Nurse Practitioner in a primary care setting
- SystmOne Experience
- Excellent communication and interpersonal skills
- Ability to work effectively as part of a multidisciplinary team
- Knowledge of current NHS policies and procedures
- Interest in Women's Health
