This is a full-time, permanent role with hybrid working, and a salary of £24,895 - £28,132 per annum.

This is an excellent opportunity for an enthusiastic Social Care Practitioner to join our Front Door Team.

The Role

The Social Care Practitioner is part of the Front Door Team, the initial contact point for residents seeking social services for the first time. The team provides advice, signposting to services, referrals, occupational therapy equipment, and conducts statutory assessments in line with the Care Act 2014.

Your Responsibilities

• Conduct assessments and interventions with residents
• Signpost to other services and provide advice to residents
• Manage and hold a caseload

Candidate Requirements

• Experience or background working with people in a care setting or similar
• Good IT skills, including MS Office, Word, and Excel
• UK driving licence
