Overview

Are you looking for a hybrid role with 2 days in the office and 3 days visiting clients or home based? Do you have experience as a Care Manager? Active Care Group's Care in the Home division continues to grow, delivering safe, reliable and outcome-based care to clients with the support of our team of clinicians, support workers and management.

We have an opportunity for an experienced Care Manager with NVQ 4 or above in Health and Social Care to join our friendly and supportive team in Oxford and surrounding areas. This role offers ownership of care packages and the rewarding impact of helping clients remain in their own homes for as long as possible.

Active Care Group has over 25 years of specialist experience providing high-quality complex care in clients\' homes, taking into account medical and social needs as well as emotional wellbeing. Our packages are tailored to fit each person\'s needs, preferences and lifestyle, and we consider the wider impact on families and friends.

What you\'ll be working

  • This is a hybrid role with 2 days in the office (Thame, Oxford) and 3 days a combination of client visits and/or home-based work
  • 37.5 hours per week, predominantly Monday–Friday 9:00am–5:30pm (hours/days may flex); you must be willing to be part of an on-call rota on nights/weekends

What you\'ll be doing

  • Own and manage care packages in your area
  • Work with the Business Development Manager to review suitable referrals
  • Produce suitable assessment reports for care packages for clients
  • Build excellent working relationships with clients, families and external bodies
  • Ensure we provide client-focused high-quality care at all times
  • Support with recruitment and training for Support Workers for care packages in your area
  • Ensure all records are accurate and completed in a timely manner

What you\'ll have

  • NVQ 4 or above in Health and Social Care
  • Full UK Driving Licence
  • A passion for delivering high-quality client-focused care
  • Experience in care at Team Leader level or above; previous management experience would be an advantage
  • Excellent verbal and written communication skills
  • A positive attitude and a commitment to continuous improvement and quality
